Add link to ticket to Github issue

A link back to the ticket instead of just the customer name / email would speed up the workflow.

We have a branch for this ready for a PR but I'll wait for this one to be merged before creating the PR so that you won't have to review most of the same changes twice (the changes for the file structure which are the ones I used to root both branches)
